1.) Do I need to make an appointment?
An appointment is not necessary, walk-ins are always welcome! This enables you to be seen at your convenience.
2.) Do you accept my insurance?
Hazel Park Urgent Care is a participating provider with many insurance plans including Medicare, Medicaid, BCBS, PHP, Aetna, PPOM, IHP, and many others. In the rare event that we do not participate directly with your insurance plan, we can submit your claim directly to your U.S. based insurance company on your behalf.

4.) What are the payment policies? What payment methods are accepted?
Co-Payments are due prior to being seen. Hazel Park Urgent Care will submit claims to your insurance company for payment. Any claims that are unpaid by your insurer are ultimately your responsibility. We accept cash, credit card, debit card, and most insurances.
