Do I need to make an appointment?
An appointment is not necessary, walk-ins are always welcome! This enables you to be seen at your convenience.
Do you accept my insurance?
Hazel Park Urgent Care is a participating provider with many insurance plans including Medicare, Medicaid, BCBS, PHP, Aetna, PPOM, IHP, and many others. In the rare event that we do not participate directly with your insurance plan, we can submit your claim directly to your U.S. based insurance company on your behalf.

What do I need to bring?
Please bring an active insurance card, driver’s license/photo ID along with any important medical information. Please be sure to let our staff know of any allergies, current medications, and your medical history.
What are the payment policies? What payment methods are accepted?
Co-Payments are due prior to being seen. Hazel Park Urgent Care will submit claims to your insurance company for payment. If you have an unmet deductible, we will collect a $90 estimated payment toward your visit prior to being seen. We will bill you any remaining balance once your insurance processes your claim. Any claims that are unpaid by your insurer are ultimately your responsibility. We accept cash, credit card, debit card, and most insurances.
When is Hazel Park Urgent Care not right for me?
If you feel that your condition is life-threatening, you need to be seen in a regular Emergency Room as soon as possible. Please visit the bottom of the ‘Services’ page for more information.
